Official Championship Rosters:
A Championship Roster created by our online Membership/Registration Program is required for all State Championship Play. After you have submitted and paid for your Membership/Registration your local commissioner will approve your roster and the Online Program will email your Championship Roster to your team administrator. Online Registration for Youth teams is located in the Youth Program Section, and Registration for Adult teams is located in the Adult Program Section of our website.
The Championship roster is a three page document:
Page 1 of 3 contains your Waver and Liability Statement and must be signed by the team manager.
Page 2 of 3 lists all of your team players and must be signed by a Parent/Guardian for youth players, or by the player for adult players.
Page 3 of 3 lists the team manager and all coaches and must be signed by the manager and coaches.
It is the responsibility of the team manager or coach to hand carry their Championship Roster to their State Championship check-in.
The Team Name, Team ID Number, and Classification is listed on the top line for page 2 of 3 of your Championship Roster.
Do Not confuse your online team menu that lists your team members for your team’s Championship Roster.
